Choose your Rank, Status and fill in your Rate (like QM) if you are/were enlisted. People looking for you remember you as a BM (or whatever), and probably won't know your current rank. If you were both Officer and Enlisted, choose Both in the drop down field and fill in your Rate. If you had multiple Rates, put them all in the Rate field separated by a /, i.e. QM/AH.

For the Personal Web Page, just add one if it's about you, not the company where you work. If you own your own company, and you have a web site, that can be entered. NO COMMAND WEBSITES PLEASE!

Use the Commands block to add in past command names (use the short name like Koelsch for example, instead of USS Koelsch FF-1049).

WHY - Command email addresses and work email addresses are likely to change which would lessen the chance that you can be located by a shipmate. Also, I check the validity of email addresses on a periodic basis and using your personal email address as your primary here lessens the chances of your address returning as invalid.
